EOPYY’s single certificate for special-education therapies

From 1 July 2026, the certificate for special-education therapies follows one EOPYY template. What the form asks for, and what the clinic keeps ready.

What EOPYY published

On 22 June 2026 EOPYY published a notice on a single certificate form for special-education therapies. The notice rests on circular ΔΒ3A/Φ115/4/οικ.14970 of 17 June 2026, from the Directorate General for Organisation and Planning of the Health-Services Market.

The circular points back to the documents beneficiaries submit with the electronic claim for reimbursement of special therapies under article 45 of the EKPY. Among other items, the digital claim must include a certificate from the therapist or the owner of the centre.

What the certificate has to state

The certificate lists the dates the therapies took place and the therapists’ details: full name, specialty, and professional licence. It is signed by the therapist or the centre owner, and by the therapists who delivered the sessions.

So that centres do not invent their own layouts, EOPYY attaches a template. The form goes in with the other required documents, whether the claim is filed through the health-insurance file or in person.

When it becomes mandatory

Completing and submitting the form is mandatory for special-education therapies carried out from 1 July 2026 onward. Sessions before that date do not fall under the rule only because a family is filing now.

For an owner, the date matters twice: whether the form is required, and which sessions belong in the period the certificate covers.

Fields on the template

The template certifies that the child attended therapies in a stated period and has a place for the AMKA. Separate blocks cover speech therapy, occupational therapy, psychotherapy, physiotherapy, and special education.

Each block asks for the therapy dates, the therapist’s details, stamp and signature, licence number, the authority that issued the licence, and a date. Physiotherapy also asks whether the sessions were at home: yes or no.

It closes with a line that the sessions took place and are certified by the therapist who delivered them, plus a place for the centre’s legal representative or the therapist.

What this means for the clinic

The practical point is not the legal phrasing. It is whether reception can, when a family asks, find the date, the type of therapy, and the therapist without rebuilding the month from notebooks and messages.

Children’s names, AMKA numbers, and licence numbers stay on the form you give the family. They do not belong in a post, a screenshot, or on this public website.
