What a clinic keeps ready for the therapy certificate

A readiness list for owners: dates by therapy type, therapist details, and signatures, without putting personal data on a public channel.

Why readiness matters before anyone asks

From 1 July 2026 the single EOPYY certificate is mandatory for special-education therapies carried out from that date on. Families often ask when the month closes, not when reception has a gap.

This page is a list for the owner and reception. The news, the circular date, and the template fields are written up separately, with a link to the EOPYY notice.

Dates split by therapy type

The template does not ask for a general line that the child “attended therapy”. It asks for dates in separate blocks: speech therapy, occupational therapy, psychotherapy, physiotherapy, and special education.

A moved appointment has to show the date that remained, not the one that was cancelled. The period the certificate covers should be the same across the blocks you complete for that claim.

Physiotherapy also asks whether sessions were at home, yes or no. If the centre does both, that distinction cannot live only in the therapist’s memory.

Therapist details and signatures

For each therapist who delivered sessions, the template has a place for details, stamp, signature, professional licence number, the authority that issued it, and a date.

The circular asks for a signature from the therapist or the centre owner and from the therapists who delivered the sessions. The template also has a place for the centre’s legal representative or the therapist. Decide in advance who signs which role, so you are not hunting for a stamp while a family waits.

Keep a copy of the licence number in the colleague’s file, not in a shared message and not on a public page.

What stays off public channels

A child’s name, AMKA, diagnosis, and licence number do not go into a post, a parent group chat, or a screenshot of the schedule.

Where scheduling software stops

The certificate is easier when the clinic day already shows which therapist had the session and which date remained after a change. The gap is usually not the stamp. It is the date lost in a reschedule.

Calendo Health keeps the schedule and coordination for therapy clinics. It does not fill in the EOPYY form and it does not send a reimbursement claim. The centre checks the certificate against the current circular. If the template changes, the file on the EOPYY website is the one that counts.
